How could you address a high school student who fails to turn their CICO form on a regular basis?
A lot depends on the reason the student is failing to turn the form in. If they are just refusing to because they don't want to, there may be little you can do. In this case, you might try connecting an incentive to turning the form in, like a freebie at lunch from the lunch line, getting to cut in to the front of the lunch line, getting free time to do something they like to do, or giving them a coffee from the teacher lounge or office to take to first hour class and drink every morning they have turned their sheet in the previous afternoon. Or whatever is an incentive for that student.
If the student is failing to turn it in due to being forgetful, I'd suggest taking the paper component out of the equation and doing the form online digitally. This might double as being an incentive as well as removing the need to remember a paper form everyday. This could be done in numerous ways. You would just need some way for the student and person doing the CICO to both have access to this form. Google Docs is a good way to collaborate on a doc like this.
