Purposeful Incontinence

I have a client who is having issues with Purposeful Incontinence (BM). I am at a loss as for interventions. I have determined the primary maintaining variable for the behavior to be attention-seeking, however, it is happening even when he is receiving a lot of attention. We have ruled out medical causes and believe this to be a behavioral issue. Thanks in advance for any suggestions you can offer me.

I have a student who has the same issue. He refuses to accept responsibility for his actions and will only change his clothing when sent to the office. Even then, he tries avoidance tactics and would prefer to stay in his soiled clothes. I would be interested in suggestions as well.
